Sensorineural hearing loss occurs when sensory cells in the inner ear or nerve pathways carrying sound information toward the brain are damaged or do not function normally. It is the most common broad type of permanent hearing loss. It may affect one or both ears, develop gradually, be present at birth, or appear suddenly.

How sensorineural hearing loss develops

Sound travels through the ear canal, eardrum, and middle-ear bones before reaching the cochlea in the inner ear. In the cochlea, specialized sensory hair cells convert vibration into signals that travel through the auditory nerve toward the brain.

Sensorineural hearing loss can develop when cochlear hair cells are damaged or lost, other inner-ear structures do not function normally, auditory-nerve fibers are affected, or the auditory system developed differently. Human cochlear hair cells do not naturally regrow after they are destroyed.

Sensorineural versus conductive hearing loss

Sensorineural loss involves the inner ear or auditory nerve and may affect speech clarity as well as loudness. Conductive loss involves the outer or middle ear and often reduces the level of sound reaching the inner ear. Mixed hearing loss includes both components.

Sudden sensorineural hearing loss

Sudden sensorineural hearing loss develops rapidly, often in one ear. It may be noticed upon waking, during a telephone call, or with a sudden pop. Fullness, tinnitus, or dizziness may occur.

Hearing that drops suddenly over hours or a few days requires immediate medical evaluation. It can feel like congestion, pressure, or a blocked ear, but do not wait to see whether it clears or rely on online screening.

How it is diagnosed

A comprehensive evaluation considers the onset and progression of symptoms, communication difficulty, tinnitus, dizziness, noise exposure, medication exposure, family history, and medical conditions. Otoscopy checks the ear canal and visible eardrum.

Air-conduction testing measures hearing through earphones, while bone-conduction testing bypasses the outer and middle ear. In a typical sensorineural pattern, both are reduced similarly without a clinically significant air–bone gap.

Speech testing, otoacoustic emissions, auditory brainstem response testing, imaging, or other medical tests may be recommended depending on the hearing pattern and symptoms.

Is sensorineural hearing loss permanent?

Most chronic sensorineural hearing loss is permanent. Hearing technology does not repair inner-ear cells, but it can improve access to sound by delivering an adjusted signal to the remaining hearing system.

Sudden loss is different: some people recover partially or fully, spontaneously or with treatment, but recovery cannot be predicted. Follow-up testing can monitor stable, progressive, or fluctuating patterns.

Treatment and management

Hearing aids are a primary option for many people. Good care includes comprehensive testing, device selection based on daily needs, individual programming, verification when appropriate, orientation, counseling, and follow-up adjustments.

Cochlear implants may be considered when appropriately fitted hearing aids do not provide enough access to speech. Assistive options such as remote microphones, television systems, captions, and alerting devices can help in specific listening situations.

Over-the-counter hearing aids are intended for adults with perceived mild-to-moderate hearing difficulty, not children or every hearing pattern. Sudden loss, significant asymmetry, pain, drainage, or severe dizziness need medical evaluation.

Can sensorineural hearing loss be prevented?

Not every cause is preventable, but noise-related risk can be reduced. Turn down personal listening volume, move farther from hazardous sound, limit exposure time, and wear correctly fitted hearing protection when loud sound cannot be avoided.

Protection still matters after hearing loss is present because additional damage can occur. Monitoring may also be appropriate when medications could affect hearing.

Sensorineural hearing loss in children

Children may be born with sensorineural hearing loss or develop it later. Early access to communication supports language, learning, and social development.

Care may involve pediatric diagnostic testing, ongoing monitoring, hearing aids, cochlear-implant evaluation, remote-microphone technology, early intervention, school accommodations, and family-centered communication planning.

When to seek urgent medical care

Seek immediate medical evaluation for hearing that drops suddenly, a change after head or acoustic trauma, sudden change with tinnitus, fullness, or dizziness, facial weakness or numbness, rapidly progressive loss, or severe pain or drainage. Call emergency services for possible stroke symptoms.
