Conductive hearing loss occurs when sound cannot travel efficiently through the outer or middle ear to reach the inner ear. It can make sound seem quieter, blocked, or muffled—similar to listening with an earplug in place. Some causes are temporary and medically treatable, while others may require ongoing care, surgery, or hearing technology.
How hearing normally travels through the ear
Sound enters the ear canal and vibrates the eardrum. Three tiny middle-ear bones—the malleus, incus, and stapes—transfer that vibration toward the fluid-filled cochlea in the inner ear, which converts it into signals for the auditory nerve and brain.
Conductive hearing loss develops when something interferes with sound movement before it reaches a functioning inner ear. The issue can involve the outer ear, ear canal, eardrum, air-filled middle-ear space, or middle-ear bones.
Conductive versus sensorineural hearing loss
Conductive loss involves the outer or middle ear and often reduces sound level. Sensorineural hearing loss involves the inner ear or auditory nerve and can affect both sound level and speech clarity. Mixed hearing loss includes both components.
Symptoms cannot reliably identify the type of hearing loss. A comprehensive evaluation compares the hearing pathway through the ear with bone-conduction testing that bypasses the outer and middle ear.
What does conductive hearing loss feel like?
Symptoms vary with the cause and whether one or both ears are affected. Some people notice that speech improves when the volume is increased because inner-ear clarity may remain relatively good, but that pattern alone cannot confirm a diagnosis.
- Muffled or reduced hearing, a plugged feeling, or ear pressure
- Hearing your own voice differently, tinnitus, ear pain, fluid, or drainage
- Difficulty with soft speech or hearing better in one ear
- Changes after a cold, flight, diving, swimming, or water exposure
Common causes in the outer ear
Earwax can accumulate, harden, and block the ear canal. Do not insert cotton swabs, tools, or ear candles; they can push wax deeper or injure the ear. The safest removal method depends on your ear and health history.
A foreign object, canal swelling, infection, or a structural difference can also reduce sound transmission. Do not probe for an object; a battery in the ear requires immediate emergency evaluation. Pain, drainage, or redness should be assessed by a clinician.
Common causes in the middle ear
Fluid behind the eardrum, middle-ear infection, and Eustachian-tube dysfunction can limit eardrum movement and create pressure or a blocked feeling. Persistent or one-sided fluid in an adult deserves medical evaluation.
An eardrum perforation, problems with the middle-ear bones, otosclerosis, or cholesteatoma can also cause conductive or mixed loss. Management may range from observation and medical treatment to hearing technology or ENT-directed surgery.
How conductive hearing loss is diagnosed
An evaluation generally starts with a health and hearing history and otoscopy, which lets a clinician inspect the canal and visible eardrum for wax, swelling, drainage, a foreign object, or a perforation.
Audiologists measure air-conduction thresholds through earphones and bone-conduction thresholds through vibration. When bone-conduction thresholds are meaningfully better than air-conduction thresholds, an air–bone gap can indicate a conductive component.
Tympanometry briefly changes ear-canal pressure and measures eardrum movement. It can provide information about pressure, fluid, mobility, perforation, or ear-tube status. Additional testing or medical imaging may be selected based on the findings.
Is conductive hearing loss permanent?
It depends on the cause. Hearing may improve after safe wax removal, resolution of middle-ear fluid, or treatment of infection. Eardrum and middle-ear-bone problems may improve with surgery, while some structural conditions are managed with conventional or bone-conduction technology.
The amount of recovery cannot be predicted without an evaluation. Even a treatable cause may leave ongoing difficulty when damage is extensive or an inner-ear component is also present.
Treatment options
Medical treatment may address infection, inflammation, pressure-related problems, or another underlying condition. Antibiotics, decongestants, and ear drops are not appropriate for every cause, so treatment should follow an examination.
When medical treatment is unavailable, incomplete, or not preferred, a properly fit conventional hearing aid may improve access to sound. Bone-conduction systems bypass the outer and middle ear and may be considered for selected conductive or mixed losses. A trial may help demonstrate benefit before an implanted option is considered.
Remote microphones, television systems, captioned telephone services, and alerting devices may also support specific listening situations.
Conductive hearing loss in children
Temporary conductive hearing loss from middle-ear fluid is common in children. Fluctuating hearing can still affect access to speech in classrooms and during important stages of language development.
Children with persistent fluid, recurrent infection, delayed speech, failed hearing screening, or listening concerns should receive appropriate medical and audiologic follow-up. Care may involve a pediatrician, ENT, pediatric audiologist, school team, and speech-language pathologist.
When to seek urgent care
Seek immediate medical evaluation for hearing that drops suddenly over hours or a few days, even if the ear feels blocked. Sudden sensorineural hearing loss can resemble wax, congestion, or pressure and may require time-sensitive treatment.
- Hearing change after head or ear trauma
- A battery or sharp object in the ear
- Severe pain, swelling, concerning drainage, or severe dizziness
- Facial weakness, numbness, fever with significant ear symptoms, or possible stroke symptoms

Some causes can be corrected or substantially improved, including certain wax blockages, infections, fluid, or structural problems. Outcomes depend on the cause and the hearing system.
Is conductive hearing loss the same as a blocked ear?
Not exactly. Conductive loss can create a blocked sensation, but sudden inner-ear hearing loss can feel similar. A rapid new change needs immediate medical evaluation.
Can earwax cause conductive hearing loss?
