Hearing Loss in Both Ears

Hearing loss that affects both ears is called bilateral hearing loss. The amount and pattern of hearing loss may be similar in each ear, or one ear may hear better than the other.
What does bilateral hearing loss mean?
“Bilateral” simply means that both ears are affected. It does not describe how severe the loss is or whether the ears are identical.
An audiologist may describe bilateral hearing loss as:

### Sound direction

Some people are born with hearing loss in both ears, while genetically influenced hearing loss can appear later or progress over time. It may occur by itself or as part of a broader medical condition.
Family history is useful information, but the absence of known hearing loss in relatives does not rule out a genetic contribution.
### Medication-related hearing changes
Certain medications can affect hearing or balance. The risk depends on the medication, dose, treatment duration, other health factors and individual susceptibility.
An audiologist may perform baseline and follow-up testing when ototoxic exposure is a concern. Never stop or change prescribed medication without speaking with the prescribing clinician.
### Outer- and middle-ear conditions
Earwax, middle-ear fluid, infection, eardrum problems or changes involving the middle-ear bones can reduce sound transmission in both ears.
Some conductive causes may improve with medical treatment. An ear examination and air- and bone-conduction testing help distinguish these patterns from inner-ear hearing loss.
### Illness, injury and other health conditions
Infections, autoimmune disease, head injury and other conditions can sometimes affect hearing. Diabetes, circulation problems and other chronic health factors have also been associated with hearing difficulty, but association does not establish the cause in an individual.

Sudden versus gradual hearing loss
Gradual bilateral hearing loss commonly becomes noticeable over months or years. It deserves professional evaluation but is usually scheduled as a routine appointment.
Sudden hearing loss is different. A rapid drop over hours or a few days—whether in one ear or both—requires immediate medical evaluation. Do not assume that both ears suddenly feel blocked because of allergies or congestion.
Sudden bilateral sensorineural hearing loss is uncommon and may be associated with systemic or neurological conditions. Seek care immediately rather than waiting for an online screening or routine hearing-aid visit.

### Ear examination
The ear canals and visible eardrums are inspected for wax, irritation, drainage or structural findings.
### Pure-tone testing
Air-conduction testing measures the softest tones heard through earphones. Bone-conduction testing bypasses the outer and middle ear. Comparing the results helps identify conductive, sensorineural or mixed hearing loss.
### Speech testing
Speech testing evaluates the softest level at which speech is detected or recognized and how clearly words are understood under controlled conditions.
Speech-in-noise testing may be added when difficulty in restaurants, meetings or groups is a major concern.
### Middle-ear and objective tests
Tympanometry, acoustic reflex testing, otoacoustic emissions or auditory brainstem response testing may be selected depending on the symptoms and clinical question.
Understanding whether the ears are equal
Both ears can have hearing loss without having identical results. The audiologist will compare:
- Hearing thresholds across frequencies - The type of loss - Speech-recognition ability - Middle-ear findings - Changes from previous tests
An unexpected or significant difference may lead to ENT referral or other evaluation. The decision depends on the complete pattern and symptoms, not simply a small numerical difference.
Treatment and management options
There is no single treatment for bilateral hearing loss. Recommendations should reflect the cause, degree of loss, speech understanding, communication goals, health considerations and preferences.
### Medical treatment
Conductive loss caused by wax, fluid, infection or another outer- or middle-ear condition may be medically treatable. An ENT or another qualified clinician manages medication or surgery when needed.
Most permanent sensorineural hearing loss is managed through hearing technology, rehabilitation and communication strategies rather than medication.
### Two hearing aids
When both ears have aidable hearing loss, two hearing aids are commonly considered. The American Academy of Audiology notes that, for people whose two ears can benefit, two devices can offer advantages for sound quality, word understanding and listening in noise.
Potential benefits may include:
- More balanced access to sound - Better awareness from both sides - Improved speech understanding in some environments - Support for sound localization - Reduced need to turn one ear toward the speaker
Two hearing aids do not restore normal hearing, and outcomes vary. Appropriate programming, real-ear verification, counseling and follow-up matter as much as the device label.
### One hearing aid when both ears have loss
Some people consider one device because of cost, comfort, dexterity or preference. If both ears are candidates, ask the audiologist how one versus two devices may affect your specific goals.

### Cochlear implants
Cochlear implants may be considered when appropriately fitted hearing aids do not provide enough access to speech for a person with severe hearing loss.

Technology works best as part of a broader communication plan.

- Face the speaker in good lighting - Reduce background noise - Move closer to the conversation - Ask one person to speak at a time - Rephrase rather than repeatedly saying the same words - Use captions and written confirmation - Discuss communication needs with family and coworkers
Auditory rehabilitation may include structured listening practice, device orientation, communication-partner counseling and self-advocacy.
Hearing protection after hearing loss is identified
Existing hearing loss does not make the ears immune to additional damage.
- Lower volume when possible - Move away from hazardous sound - Use activity-appropriate earplugs or earmuffs - Avoid combining multiple loud exposures without recovery time - Follow workplace hearing-conservation requirements - Monitor hearing as recommended
Avoid wearing hearing protection continuously in ordinary safe environments unless advised. Overprotection can interfere with communication and environmental awareness.
